Help filling out a Money Gram?

I have a money gram that I am suppose to take to the bank and cash and send money by Western Union. I know my name goes in the pay to but what do I put in the purchaser and whose address do I put in the address section? I usually deal with money orders. This money gram is payable to me. Thanks
